Senior Leadership Conference Form

Congratulations!  The summer is near and your senior year is in sight.  It is now time to sign up for the Senior Leadership Conference.  This year's conference will be held at Christian Brothers University, with additional activities taking place at BRIDGES.  Attendance is mandatory for one of the 4 weeks of the Conference.  To optimize your chance of receiving the week you would like to attend, please fill out this form as soon as possible.  A confirmation packet that includes your assigned session, a permission form, a participant guide, and a "What to Bring" list will be given to you and your parents during the MANDATORY parent meeting in the month of May.  You will recieve information about this meeting in March. 

Please remember that you must attain (6) meeting credits to attend Senior Conference.  Failure to complete these requirements disqualifies you for Senior Conference even if your forms are completed.

Please rank order the weeks 1, 2, 3, and 4 (with 1 being your first choice) as to which week you would like to attend Conference.  If you absolutely cannot attend a week, please indicate by choosing "Cannot Attend."
